About the NDIS

The National Disability Insurance Scheme (NDIS) is the new way of providing support for Australians with disability, their families and carers.

We will work with you to identify supports you need to live your life. Supports may help you achieve goals such as independence, health and well being, improved learning and improved daily living.

The NDIS gives you more choice and control over how, when and where your supports are provided, and gives you certainty you will receive the support you need over your lifetime.

Our tailored plans

Once the NDIS has confirmed you meet the access requirements of the scheme, one of our dedicated planners will arrange to meet with you.

You will then be able to discuss your needs, goals, what you are looking for and the supports you currently receive.

Your personal planner has all the necessary information, to tailor a plan of supports that will best meet your needs and goals.

One-on-one support

Our friendly and experienced client service officers can assist you with the following:

  • Learning about the NDIS
  • Outlining your needs and goals
  • Implementing your supports
  • Help in collecting necessary documentation for the application process
  • Coordinating the full range of your supports

How to Access our Services

In order to access the disability services at Creating Links you must have a diagnosed disability, be a NSW resident and be aged 0-65.

If you do not have an NDIS package, are located within a ‘Year 2’ NDIS area or would like to discuss your eligibility and goal outcomes we encourage you to talk to one of our friendly staff on 02 8713 7700.

Not yet in an NDIS region?

We continue to operate services to support community participation, flexible respite and other FACS funded activities in areas not yet funded through the NDIS.
